This is the Club Registration Form which is required to be completed by a player, in full, prior to start of the season and in order to participate in competition. If the player is Under 18, this form must be filled out by a parent or legal guardian to the child. The player details supplied in this form are used to register the player with FAINet in order to make them eligible to play with the team for which they wish to register. In Order to complete registration, the player will be required to sign the FAINet form which will be provided through the club's officials All Club Rules and policies must be adhered to and are a requirement via the submission of this form
